Evaluation of the Current School Performance
Original and photocopy of the evaluation of the school performance during the First Quarter / Semestral / Trimester of the current grade level (SY 2022-2023)
-If the report card / evaluation report / progress report is not yet available, a Certificate of Enrollment (COE) may be submitted. The COE must indicate the date of availability of the report card with at least first quarter / semestral grade.
-The report card / evaluation report / progress report must also indicate the Learner's Reference Number (LRN) of the applicant.
If the child stopped schooling due to the pandemic, report card / evaluation report / progress report for the last two (2) school years attended must be submitted.
Evaluation of the Previous School Performance
Original and photocopy of the report card / evaluation report / progress report from the previous grade level (SY 2021-2022)
Recommendation Form (Xavier School Nuvali Form)

Family Picture
Two (2) copies of recent identical colored family pictures. One (1) copy is to be pasted on the Application Form and the other copy is for the Student Information Sheet, which will be released once accepted.
Note: Only immediate family members should be in the picture and sunglasses should not be worn by any person in the picture. Scanned and computerized pictures are not allowed.

Data Privacy Agreement Form
The data privacy agreement form must be signed by the parents to signify that they have read, understood, and agree with the Data Privacy Policy of the School.
Student Health Record Form
The Student Health Record Form must be filled out by the parent, your child’s
physician, and specialist (for children with special medical conditions)
Recommendation Form
The Recommendation Form should be filled out either by the school head or class adviser or guidance counselor of your child’s current school. Please forward the PDF form to the current school of your child and request them to submit the scanned copy of the accomplished form through an upload facility at https://bit.ly/recoformforxsn
Therapy Report Form (if applicable)
The therapy/assessment report from the child’s physician/therapist must be attached to the Therapy Report Form if the child has undergone any form of therapy.

Create an account and register to Xavier School Nuvali Admissions Portal at https://admissions.xsn.edu.ph/applicant/register
Registration
Encode all the required information in the system; after submission of the
required information, download, print and sign the application form from the portal.
Submission of Admission Forms and Other Documents
Upload scanned copies of the following documents to the parent’s portal
using the account created:

Submission of the Recommendation Form
Accomplished Recommendation Form of your child will be directly submitted to
Xavier School Nuvali by your child’s current school through an upload facility.
You will also be notified through the portal if the recommendation form has been
successfully submitted to us.
Confirmation of Grade Level
The grade level you are applying for your child will be confirmed by the Registrar through the parent’s portal.
Application and Testing fee payment
Instructions on how to proceed with application and testing fee payment will also be posted in the parent portal. After payment has been made, upload the scanned copy of proof of payment receipt in the parent portal. The Accounting Office will verify your payment afterwards.
Always check and monitor the status of your child’s application through the parent’s portal.
Applicants are expected to be at the testing venue at least ten (10) minutes before the scheduled test. Latecomers will not be entertained.
Testing may be rescheduled (and you will be given a new schedule through the parent portal) only in the following instances:
The applicant is sick and not well enough to take the test
There is death in the family
Xavier School calls off classes/offices due to inclement weather or other unforeseen events that could affect the scheduled testing. We will call you when this happens.
In case testing needs to be rescheduled due to reasons other than those cited above, additional testing fee will be required and a new test permit will be issued to the applicant.
To ensure a smooth and successful testing administration, the applicant must strictly follow the testing guidelines and procedures indicated in the test permit.
Release of the Application Results
Admission Result of your child will also be posted in your account through the XSN Admissions Parent Portal. The parents are encouraged to regularly check their emails too for any further announcements after the admissions test of their child. Results will not be released over the telephone.
Xavier School Nuvali reserves the right to withhold information regarding actual test scores of the applicant and the test administered is solely for the use of Xavier School Nuvali admission purposes only.
